Pain Program

You are a unique individual with unique circumstances surrounding your pain. One of our primary goals is to detect and treat the underlying root causes of your pain in order to assist you in returning to doing the things that are most meaningful in your life. Therefore, we have established a payment program for your specific needs. During your initial appointment, your therapist will provide you with a personalized plan to meet your specific needs. Included in that plan is an estimated number of sessions required to complete your therapy. 

Infertility Program

Complete resolution is often achieved in 16 sessions.  JEMPT does not accept insurance as a form of payment but we do accept most credit cards.
To ensure the best possible outcome, we recommend completing the full 16 hours within three months.

Do you take my insurance?

Joint Effort Manual Physical Therapy is a self-pay clinic. We are out of network providers for all insurance companies.

We accept check, cash, Apple Pay Cash, and health savings accounts as forms of payment.

It is our policy to help you receive maximum benefit.
We will provide you with the necessary forms to obtain reimbursement from your insurance company to the extent of your coverage.*

*The amount of reimbursement you receive will vary accounting to the terms of your insurance policy.

Insurance Information

Physical therapists are healthcare professionals. We provide physical therapy (PT) services through Florida state licensed professionals, qualified to treat pain and dysfunction. 
Below is a list of the most common PT procedure codes used for billing purposes. They are defined and approved by the American Medical Association:

  • 97163 - PT initial evaluation
  • 97110 - therapeutic procedure
  • 97112 - neuromuscular reeducation
  • 97140 - manual therapy
